Although CA Public Health Microbiologists (PHM) are not really clinical laboratorians and don’t generally work in clinical laboratories, to apply for certification, you must follow the Laboratory Field Services (LFS) on-line application process for “Clinical Laboratory Professional Licensing”. The CA laws are different, there are no fees for PHMs and there is no renewal of your certification.
See below for LFS website information using the on-line application process.
First Time Applicants:
California state law requires that licensed clinical laboratories in California employ testing personnel who are licensed by the State. The new personnel license application process requires the following steps:
Request access to the Laboratory Field Services application, update user profiles, or reset passwords.
Log in to your account, create an application, sign the attestation page, upload documents, continue an incomplete application, or check the status of an application.
*Note: Application fee must be paid using a valid Visa or MasterCard credit card.
If you have any questions or difficulties, please check our FAQs.
For information and instructions on how to renew your clinical laboratory personnel license or certificate, visit the LFS personnel renewal web page.
As of August 15, 2016, the American Association of Collegiate Registrars and Admissions Officers (AACRAO) discontinued its educational transcript evaluation services. Until further notice, LFS will accept educational transcript evaluations completed by “Current Members” of the National Association of Credential Evaluation Services (NACES), or “Endorsed Members” of the Association of International Credential Evaluators, Inc. (AICE). Evaluations completed before August 15, 2016 will only be accepted if completed by AACRAO.
